Please fill out the following form and you will be assigned an account number and address in our warehouses where we can receive your shipments or online shopping! 


Please upload a digital copy of your ID Document in PDF or image format
Please provide your current residence address, i.e: 123 Main Street  
Provide full city name and state, i.e.: Caracas, Distrito Capital  or Miami, Florida
If available
For customers living in Venezuela, we require a second document. At least one of them must have a picture of the customer (such as passport, national ID document)
Please type the code or the name of the closest or preferred TEALCA's location. You can check all the available locations at https://www.tealca.com/oficinas/
For customers living outside of Venezuela, we need the information on the consignee who will receive your packages
